UNINSURED SERVICES
Services not covered by OHIP, available for a transparent fee.
While OHIP covers most medically necessary healthcare services, some services are not funded by the provincial health insurance plan. These are known as uninsured or non-OHIP services.
At Nilab Family Practice, we provide a range of uninsured services to support our patients’ medical, administrative, and occupational health needs. Fees for these services are not covered by OHIP, are charged in accordance with Ontario Medical Association (OMA) guidelines where applicable, and will always be discussed with you in advance.
• Transfer of medical records to another healthcare provider: $50
• Requests requiring more than 20 pages of documentation are subject to an additional charge of $1 per page.
• Completion of insurance-related medical forms, including Attending Physician Statements (APS): $325. These fees are often reimbursed by the insurance company.
• Preparation of medical reports for legal matters: $250. In many cases, this expense is coordinated through legal counsel.

Driver's Medical Exam

Need a medical examination for your driver’s licence requirements? Our clinic provides professional driver fitness evaluations completed by experienced family physicians. We offer timely appointments, a detailed assessment process, and assistance with the required documentation. The cost of the examination is $200. We strive to provide a convenient, hassle-free experience and help you complete your licensing requirements efficiently.
Missed Appointment

We understand that unexpected situations can arise. As a courtesy, the first missed appointment will not be charged.
After the first missed appointment, a $50 fee will apply to any additional no-shows. A $50 fee will also be charged for cancellations made with less than 24 hours’ notice.
Any outstanding missed appointment fees must be paid before future appointments can be scheduled.
